First Visit
Each new patient is asked to complete several pieces of information before their first visit to our practice. Our office staff will be happy to mail you a new patient packet or you may find this information on our website. Please remember to bring your insurance cards and any medication that you are taking to your visit. We would suggest that you arrive 30 minutes before your first appointment to be sure all your paperwork is in order.
Routine Visits
Our goal is to provide compassionate quality care as we understand the apprehension and fear of the word cancer that brings to both the patients and their love ones. Our physicians and staff are dedicated to provide not only quality medical care, but also emotional and spiritual support. We are available to see patients by appointment from 8a.m.- 4:30p.m., Monday-Thursday and 8a.m.-12p.m. on Friday. Please remember to arrive 15 minutes before your scheduled appointment in order to us to be sure your information is in order. We request that you bring your current insurance cards and any medications with you at each visit.
Cancellation Policy
We request you call our office 24 hours in advance to cancel or reschedule an appointment so that we may offer these appointments to other patients. Failure to keep an appointment may result in a charge for the visit.
Emergency Coverage
Emergency coverage is provided for our patients 24 hours a day, every day of the year. You can reach our physician on call by calling our office phone number and when prompted enter extension 3-4-5-6.
